KTLLP/KT Connections Benefit Package

Medical Ins.

Employee – no charge
Coverage generally begins on the first of the month following 30 days of employment.

Family $285.00 month
(2010 premium)
If you and your dependents are provided medical insurance through another plan, an annual payment of $200 per month vested at the end of that year will be provided for the employee works up until December 31.

$20.00 office visit
Prescription coverage- Employee pays 100% of prescription. After deductible has been met First Administrators will reimburse employee 80% of cost when within Network.

Supplemental Life Ins. Policy for all full-time employees.
30- day waiting period, for approximately $5,000.00.

Basic Life Ins.

Firm pays for term life insurance - Employee coverage is double the employee’s annual salary. Six month waiting period of continued full time service.

Employees Spouse- ¼ of insured’s life ins. Amount up to $20,000.00

Each eligible child age 6 months to 21 years - $4,000.00
(less than 6 months-$400.00).

Long-Term Disability Income Plan

Employee – 30 day waiting period for coverage 60% of current salary after 90 days of disability due to an accident or sickness.

Short-Term Disability Income Plan

After 30 days of employment Employee may choose to participate in group plan. Insurance Co. rep will contact employee.

Hospitalized Sick Leave

Accrues at the rate of 4 hours per month of full-time employment.
Accident or illness requiring hospitalization or treatment in similar care facility will be qualified as hospitalized sick leave.

Worker’s Compensation Ins.

Firm’s expense for job-related accidents.
Employee must notify your supervisor or the Human Resources Department immediately.

Dental Insurance

Beginning of the following month of start date
Employee may choose to participate in group plan.

Retirement Plan Profit-sharing retirement plan
Every employee age 21 or older after 6 months of employment after working 1,000 hours of service
amount determined each year by the firm.
401(k) is a employee voluntary program for employees 21 years of age or older after 6 months of employment

Cafeteria Plan Employee can contribute pre-taxed dollars for Medical and child care expenses. The firm maintains a tax deferred plan through payroll deductions. The firm pays for the administrative fee. Maximums

Personal Leave Combination of hours to be used
for vacation and non-hospital sick leave
First 5 years
- 15 days per year
- 16 days for 56 hour salary persons
6-10 years
- Increases 1 day per year, up to 20 days (on January 1 of those years)
Leave without Pay

Salaried employees will be granted leave without pay upon request provided
work schedules allow and time off approved by the partner-in-charge.
The absence, granted on a full-day basis only.

Hourly employees may schedule leave without pay on an hourly basis,
provided a partner approves it in advance.

Funeral Leave Three days are granted with pay for immediate family members.
One day is granted for other relatives.
Jury Duty Compensation of regular pay for normal work hours while serving on jury duty. Provide a copy of the jury duty notice to your partner-in-charge as soon as it is received.
Military Duty Staff members fulfilling their military duty training requirements will be granted leave without pay or vacation days as the member may elect. Staff member must keep partner-in-charge fully informed of pending military service.
